RN CARE COORDINATOR

MONTANA STATE PRISON

DEER LODGE, MT

As a Registered Nurse at Montana State Prison, you will play a vital role in the medical management of over 1,600 inmates within a 24‑hour infirmary unit. This position supports comprehensive nursing functions, including assessments, triage, sick call, medication administration, mental health support, urgent and emergent response, chronic care management, infection control, quality improvement, and discharge planning.

  • Coordinate daily and weekly medical transportation schedules and prioritize patient care.

  • Maintain open communication with the medical department and off‑site clinics.

  • Liaison with contract providers, telehealth services, and off‑site clinics.

  • Complete chart reviews and schedule appointments, labs, and records.

  • Educate patients before procedures and clinic visits.

  • Facilitate informed consents and manage refusals.

  • Review all refusals for accuracy and meet with patients who decline care.

  • Monitor UM Queue and ensure appropriate scheduling.

  • Provide medication‑hold education and enter pre‑op medication orders.

  • Assist with planning post‑operative care.

  • Work with infirmary nurses on pre‑op and post‑op admissions.

  • Conduct chart reviews for referrals and locate ICD‑10 codes as needed.

  • Gather off‑site reports and monitor internal clinic follow‑ups.

  • Assist clinic staff as needed.

  • Serve as point of contact for on‑site VA appointment scheduling.

QUALIFICATIONS

The knowledge, skills and abilities for this position are typically acquired through a combination of education and experience equivalent to:

  • Associate degree in Nursing.

  • Licensed to practice as an RN in the State of Montana is required.

Alternative combinations of education and experience will be considered on a case-by-case basis.
