Position Overview

As an RN Case Manager, you will coordinate and manage patient care services, ensuring each patient receives high-quality, individualized care. You will collaborate with physicians, caregivers, and interdisciplinary healthcare teams to create and oversee effective care plans that improve patient outcomes and quality of life.

Key Responsibilities

Complete comprehensive patient assessments and ongoing evaluations
Develop, implement, and manage individualized care plans
Coordinate home health services and community resources
Monitor patient progress and adjust care plans as needed
Collaborate with physicians, therapists, caregivers, and family members
Educate patients and families on health management and wellness strategies
Maintain accurate and timely clinical documentation
Ensure compliance with all regulatory and agency standards

Qualifications

Current and active Registered Nurse (RN) license in Maine
Minimum of 2 years of nursing experience
Home health, care management, or case management experience preferred
Strong clinical assessment and critical-thinking skills
Excellent communication and organizational abilities
Experience with electronic medical records (EMR) systems
Reliable transportation and ability to travel throughout the Brunswick service area
