Their primary focus is optimizing resource utilization while ensuring effective discharge planning and patient advocacy.
- Department: Case Management Shift: Monday to Friday, 08:00 16:30 (40 hours per week) Rotation: Includes rotating weekends
- Experience Required: Minimum 2 years of recent acute care hospital discharge planning experience
- Care Coordination: Direct and coordinate patient care throughout their hospital stay, ensuring seamless transitions.
- Comprehensive Assessments: Evaluate patients' healthcare and discharge needs, identifying potential challenges and aligning care plans with covered services.
- Multidisciplinary Collaboration: Work closely with internal and external teams to develop safe and efficient discharge strategies that prevent readmissions.
- Discharge Planning Leadership: Lead the team in creating complex discharge plans that balance cost-effective hospital resource use with minimized patient expenses.
