Private Duty Clinical Team Manager (Home Health)

Full-Time | Mishawaka, IN / $5,000 Sign-On Bonus

Your Role

Oversee patient care services and clinical staff

Review and approve plans of care and physician orders

Ensure timely start of care and regulatory compliance

Conduct OASIS reviews and support quality improvement

Coach and mentor field clinicians

Partner with the Branch Director on performance metrics

Support hiring and team growth

What You Bring

Associate degree in nursing

2 plus years of RN experience in Home Health or Hospice preferred

Supervisory experience preferred

Active Indiana RN license

Experience with Homecare Homebase and Microsoft Office

Ability to travel within the coverage area

Sit, stand, bend, lift, and move intermittently and be able to lift 50 to 100 lbs
