Job Description
RN Director of Case Management Lake Dora, Florida
The RN Director of Case Management leads and supervises the department to ensure smooth Care Coordination and Care Transitions for every patient. The RN Director drives strategy and operational excellence, aligning clinical, service, and financial outcomes in a collaborative, matrixed environment. The RN Director of Case Management provides monthly metric reports and action plans to implement corporate Case Management strategies, meets regularly to align with providers and keep leadership updated, and maintains ongoing communication with the CFO for bidirectional updates on key initiatives. The Director partners with market leaders, physicians, peers, community colleagues, and system leaders to identify opportunities for continuous improvement in patient care and services while championing patient-centered care and community engagement.
Qualifications:
- Registered Nurse required
- Bachelor Degree required
- Master Degree preferred/
- Progressive Director of Case Management experience in an Acute Hospital Setting
- ACM or CCM preferred
Responsibilities:
- Provides monthly metric reports and mitigation plans to drive performance and support corporate Case Management strategies
- Oversees retention activities to ensure adequate staffing and leadership in Care Management
- Actively engages with post-acute care entities to improve patient transitions and reduce readmissions
- Leads technology change management to enhance staff efficiency, patient outcomes, and tool utilization for Care Managers
- Directs daily operations of the Care Management department, including discharge planning, care coordination, progression of care, and social services
- Ensures compliance with CMS Conditions of Participation for Discharge Planning and related federal/state regulations
- Participates in corporate Care Management meetings, cascades information, and implements strategies with standard work and processes
- Develops and oversee implementation and compliance with Care Management policies and standard operating procedures
- Collaborates with interdisciplinary hospital leaders and executives to achieve organizational goals
- Develops and meets operational budget performance while maintaining capital and fiscal accountability
- Partners with Utilization Management for the Utilization Review Committee and day-to-day operations
- Identifies improvement opportunities and develops, implements, and monitors action plans
- Educates and mentors facility Care Management staff and interdisciplinary teams on data, outcomes, and opportunities
