Job Description

RN Director of Case ManagementYakima, WashingtonThe RN Director of Case Management drives innovation in patient care and service delivery by fostering collaborative, patient-centered care practices and promoting community engagement. This leadership role oversees a dedicated team of clinical professionals, ensuring seamless Care Coordination across the Continuum. The Director of Case Management works closely with market leaders, physicians, peers, and community stakeholders to achieve organizational goals.

Qualifications:

Registered Nurse (RN) license required
Bachelor’s Degree in Nursing required
Master Degree in Nursing preferred
Director of Case Management experience in an acute hospital setting

Responsibilities:

Supervise and coordinate the work of clinical Case Managers and Social Workers
Promote quality patient outcomes while optimizing Resource Utilization
Develop and implement tools and systems for effective multidisciplinary Case Management
Identify and act on opportunities for continuous improvement in patient care and services
