Job DetailsWorking on-site at a major medical center in the Bronx NY Work Schedule is Monday thru Friday 9am to 5 pm (NO weekends ever)The RN Hospice Business Development Care Planner is responsible for evaluating and establishing a plan of care for patients referred for hospice services. The RN will facilitate admission of referrals upon discharge from the hospital to a hospice program that meets the needs of the patient and family members. As a key member of the business development and marketing team, the Hospice Care Planner develops and maintains positive relationships with physicians, social workers, case managers and discharge planners within the medical center.Interested in hearing more?
