Job Description

Trustbridge Hospice, part of Empath Health, is seeking a compassionate and dependable Registered Nurse (RN) to provide rapid response overnight field visits throughout Broward County for hospice patients & families in need of immediate support. This role is ideal for a nurse who thrives in independent practice, stays calm under pressure, and is driven by a sense of purpose, delivering skilled, responsive, in‑home care. The shift will be every Fri/Sat/Sun from 7am - 7:30pm

What you'll Do

As an RN with Trustbridge Hospice, you will play a vital role in ensuring patients and families feel supported, informed, and cared for during urgent and often emotional situations.

  • Support and uphold the organization's mission, vision, and values in every interaction
  • Coordinate patient and family care in collaboration with the attending and hospice physicians
  • Perform comprehensive patient and family assessments to identify physical, emotional, and psychosocial needs
  • Develop and manage the Interdisciplinary Group (IDG) Care Plan
  • Ensure timely implementation and coordination of all interventions and services outlined in the care plan
  • Respond promptly and effectively to emergent needs and concerns during field visits
  • Deliver a consistently high level of personalized, compassionate, and engaging care
  • Advocate for patient comfort, dignity, and quality of life at all times
  • Foster trust and reassurance to ensure the highest level of patient and family satisfaction

What You'll Need

  • RN degree from a college or university, accredited by the Accreditation Commission for Education in Nursing and/or the Commission on Collegiate Nursing Education.
  • Registered Nurse currently licensed in the State of Florida.
  • Current Basic Life Support Certification.
  • One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
  • Basic computer skills and EMR software experience a plus.
  • Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license.
