Job Description

16 hours / week (Saturday and Sunday only)Registered Nurse (RN) part time 16 hrs. per week opening in our hospice division going to facilities/nursing homes we serve for Home Health & Hospice Care. Service Areas:  Patient homes in Southern New Hampshire greater Merrimack, Nashua, Manchester communities.

Responsibilities

  • Completes an initial home health assessment of patient and family to determine hospice care needs. Provides a complete physical assessment and history of current and previous illness(es).
  • Utilizes the nursing process in planning, providing and evaluating skilled needs.
  • Completes a comprehensive patient assessment.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and family on the planning and revising process.

Qualifications

  • Graduate of an accredited school of nursing.
  • Associate degree in nursing.
  • Registered as RN with New Hampshire’s Board of Nursing.
  • A minimum of one year recent acute care experience.
  • Ability to exercise initiative and independent judgment.
  • EPIC experience preferred, willing to train! 

Preferred Qualifications

  • Bachelor degree in nursing.
  • Community Health/Education experience.
  • Certification in Hospice and Palliative Care, Oncology or end-of-life experience is preferred, but willing to train the right candidate.
  • EMR software proficiency.
  • Registered as a RN with New Hampshire’s Board of Nursing.
  • CPR Certification.
  • Satisfactory criminal record.
  • Satisfactory motor vehicle record.
  • Work one weekend a month pays additional $4.00 an hour and agency holidays 1 winter and 1 summer required pays time and a half.
