Job Title: RN – Patient Care CoordinatorJob Location: Portland, OR 97232Job Duration: 13+ Weeks | Possibility of ExtensionShift: Monday–Friday | 8:00 AM–5:00 PM | 40 Hours/WeekPay: Local: $80/hr on W2Job Summary:
- The RN Patient Care Coordinator provides comprehensive care coordination and case management support to patients, collaborating with providers, healthcare teams, and other stakeholders to ensure effective care delivery and appropriate transitions of care.
- Coordinate patient care and facilitate appropriate healthcare services.
- Assess patient needs and develop individualized care plans.
- Collaborate with physicians, nurses, patients, and multidisciplinary teams.
- Support discharge planning and continuity of care.
- Review patient records and document care coordination activities in Epic.
- Assist with Medicare and Medicaid-related patient care coordination.
- Communicate with patients, families, providers, and healthcare organizations regarding care needs.
- Monitor patient progress and follow up on identified care needs.
- Ensure compliance with organizational policies and regulatory requirements.
- Active Oregon RN, Washington RN, or Compact RN License.
- Minimum 1 year of Case Manager or Patient Care Coordinator (PCC) experience.
- Epic charting experience.
- Experience with Medicare and Medicaid.
