Job Description

Home Health Registered Nurse (RN) - Weekend Start-of-Care (SOC)

Department: Home Health

Reports To: Home Health Director of Nursing

Schedule: Weekend/ Monday Coverage (Saturday & Sunday =12hours & Monday =8 hours)

FLSA Status: Exempt (Salaried)

$7,500.00 Sign on Bonus ($3,750.00 first 6 months of employment and $3,750.00 paid out after 1 year of employment)

Employment Status: Full-Time

Position Summary

The Home Health RN - Weekend Start-of-Care (SOC) is responsible for completing comprehensive start-of-care assessments for patients admitted to home health services during weekends. This role performs OASIS assessments, initiates plans of care, ensures regulatory compliance, and facilitates a smooth transition into ongoing home health services.

Essential Duties and Responsibilities

Start-of-Care & Clinical Assessment

- Complete Start-of-Care (SOC) visits during assigned weekend shifts.

- Conduct comprehensive OASIS assessments within required timeframes.

- Perform skilled nursing assessments including physical, psychosocial, functional, and environmental evaluations.

- Complete medication reconciliation and identify potential medication concerns.

- Assess patient safety, home environment, and caregiver support.

Care Planning & Coordination

- Develop and initiate individualized plans of care in collaboration with the interdisciplinary team.

- Initiate skilled nursing services and referrals as appropriate.

- Communicate SOC findings to weekday case managers and clinical leadership.

- Provide patient and caregiver education to support self-management and safety.

Documentation & Compliance

- Document all assessments and interventions accurately in the electronic medical record.

- Ensure compliance with CMS Conditions of Participation, state regulations, and agency policies.

- Apply OASIS scoring guidelines and support quality outcome improvement.

- Meet productivity expectations for weekend SOC visits.

Professional Practice

- Exercise independent clinical judgment within established protocols.

- Maintain patient confidentiality in compliance with HIPAA.

- Participate in required compliance training and quality improvement initiatives.

- Collaborate effectively with interdisciplinary team members.

Qualifications

Required

- Active Registered Nurse (RN) license in the state of practice.

- Clinical nursing experience (home health preferred).

- Knowledge of OASIS assessments and home health regulations.

- Strong time-management, organizational, and communication skills.

- Valid driver's license, reliable transportation, and current auto insurance.

- Current BLS/CPR certification.

- Dependability and reliable weekend coverage are required.

Preferred

- Prior Home Health Start-of-Care experience.

- Experience with home health electronic health records.

Physical and Work Requirements

Ability to travel to patient homes during weekend shifts, lift or assist patients as needed, and perform nursing duties requiring standing, walking, and sitting for extended periods.

Visiting Nurses requires a criminal background check and motor vehicle (MVR) check for all employees once a conditional offer of employment has been extended by the hiring manager.
