Seeking a Coordinator for (PDN) Private Duty Nursing Services for the Independence, MO office!

Job Duties:

  • Understands and adheres to established Phoenix Home Care & Hospice policies and procedures.
  • Creates and maintains Nursing schedules.
  • Schedules Nurses according to care plans and staff availability.
  • Discerns client services required as outlined in agreements, urgent requests and care plans.
  • Enters staff and client information into database.
  • Maintains staff and client database
  • And more!

Requirements

  • Troubleshooting problems and providing resolution 
  • Ability to handle a multi-line phone system 
  • Highly Organized & Detail Oriented 
  • Previous Care Coordination a plus!
