SALARY: The position range is $223,944 annual - $280,216 annual, based on experience and qualifications.

The Site Medical Director provides clinical administrative leadership to Howard Brown Health Center. In this role, the Site Medical Director is responsible for the quality of care provided to participants in Primary Care at the assigned site, and for the clinical productivity of the medical provider clinical site staff. The Site Medical Director is a licensed professional prepared in a specialty residency program who manages actual and potential health problems based on the area of specialty. The health conditions include common, complex and/or advanced diseases needing intensive medical supervision of the prescribed treatment plan. The Site Medical Director practices independently and interdependently by collaborating with the advanced practice clinicians, community health nurses and other health care providers. The Site Medical Director is ultimately responsible for the delivery of medical care within the legal parameters established by the Illinois Medical Practice Act. The Site Medical Director also provides direct patient care, and as Physician, the Site Medical Director provides quality primary health care services to special populations, emphasizing the prevention and treatment of disease. The role of the Physician in a community setting is a response to society's demand for accessible quality health care to all populations regardless of socio-economic status. Individual accountabilities and work volume will be established through the development of annual Success Objectives, within the framework outlined below.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

As clinical administrative leader:

  • Measures and monitors quality of patient care services in Primary Care Services at the assigned clinical location, primarily the care offered by HBHC providers, including Physicians, Advanced Practice Nurses, and Physician Assistants. Meets regularly with providers to review care standards and performance approximately 2 one on one sessions per direct report per month.
  • Directly supervises Physicians, Physician Assistants and Advanced Practice Nurses practicing at the site.
  • Measures and monitors clinical productivity of provider staff, including Physicians, Physician Assistants and Advanced Practice Nurses. Develops productivity standards along with the Chief Medical Officer and reports regularly on performance relative to standards set by HBHC. Meets regularly with providers to review standards and performance.
  • Collaborates with the Integrated Health Care team in program planning, monitoring and evaluation of primary health care services including implementation of Patient Centered Medical Home and Meaningful Use at the clinical site and clinical quality initiatives.
  • Provides direct care for patients at the site for 28 clinical contact hours and 12 administrative hours per week for total of 40 hours per week.
  • Attends and participates in the Medical Leadership weekly meeting, and other leadership meetings as required

As Physician:

Facilitates entry of the patient into the health care system, either at the HBHC main clinic site, homes or community shelters or the hospitals where the Medical Director has admitting privileges by assessing the primary health care needs, including eliciting comprehensive health histories, performing physical examinations, ordering and/or performing pertinent diagnostic tests, analyzing collected data to determine client health status and formulating a problem list (diagnosis.)

Provides coordinated, continuous and culturally competent primary health care to clients that span the health continuum through mutual goal setting and planning delivery of care.

Prescribes a variety of treatment modalities to meet health and illness needs.

Meets provider productivity standard including but not limited to number of visits per week

Collaborates with advanced practice nurses and community health nurses to assure that health teaching and counseling is provided.

Will follow role assignments that are in alignment with the Patient Centered Medical Home Standards, and participate in all Patient Centered Medical Home efforts.

Meets productivity standards including but not limited to number of visits per week

Manages and evaluates the plan of care, process and outcome and then modifies the plan as needed.

Adheres to collaborative practice principles of respect and joint decision making when: ◦Providing clinical supervision of the medical care delivered by the advanced practice nurses.

Consulting with health team members about managing the client treatment plan.

Following clinical practice standards (protocols) related to each health care providers role and responsibilities.

Assures access and continuity of care by providing call for after-hours coverage. Assures continuity by obtaining hospital privileges at designated hospitals and by making home visits when appropriate.

Collaborates with other multidisciplinary professionals and community agencies to provide high quality care.

Seeks opportunities for community and professional outreach to maintain and increase patient referral base. Outreach activities can include, but are not limited to:

Providing training and presentations on health care issues to multi-disciplinary professionals and community agencies.

Attend community health fairs.

Disseminate information about health center and services provided to other agencies or health care professionals.

Coordinates services in conjunction with other team members to meet patient's primary health care needs and to ensure the continuity of care.

Participates in quality improvement activities through protocol development and collaborative peer review by: ◦Developing medical and joint practice protocols and identifying appropriate evaluative criteria for utilization review activities.

Evaluating care based on clinical standards through the quality improvement process. Implements recommended changes and/or improvements to guarantee quality care based on the evaluative process.

Assumes the responsibility for professional development and contributes to the professional growth of others by:

o Developing in-depth knowledge of daily program operations, policies and procedures.

o Acting as a preceptor in the orientation of new health care personnel, interns, residents and volunteers.

o Serving as a consultant in specialized populations to other health care providers and community agencies.

o Incorporates and disseminates research findings as a basis for practice and to improve health care outcomes.

o Identifies clinical questions to be researched.

Assumes the responsibility for maintaining competence in clinical practice by:

o Engaging in mentor relationships with experienced mid-level providers and physicians to gain clinical expertise.

o Developing and maintaining a patient panel as clinical competence is achieved to enhance continuity of care and meet Organization productivity standards.

o Participating in appropriate educational activities in order to maintain Board certification and continue professional growth.

Provides cost effective Organization productivity requirements by practicing collaboratively with the advanced practice nurses and the community health nurses to prevent the duplication of services.

Practices within the legal and ethical guidelines established by the Medical Practice Act, the Physicians Code of Ethics and the Specialty Standards of Practice.

Documents comprehensive, accurate and continual data on clients' records and program reports according to Organization standards.

Prepares the appropriate program reports to fulfill finding requirements as assigned.

Adheres to professional standards as outlined by governmental bodies, American Medical Association (AMA), American College of Obstetrics and Gynecology (ACOG), American College of Physicians, American Academy of Pediatrics, and American Academy of Family Physicians, private funding sources, Organization plans/policies and Unit guidelines.

Participates in periodic evaluative reviews and/or in-house and external staff training to ensure that he/she understands and continues to adhere to such standards. Initiates requests for assistance or guidance from Manager to address new issues or complex concepts affecting adherence to professional standards.

Performs other duties as necessary.

SUPERVISORY RESPONSIBILITIES:

The Medical Director supervises the Physicians and Mid-Level Providers and other staff as needed for the assigned clinical site.

QUALIFICATIONS, KNOWLEDGE, SKILL REQUIRED:

Education and/or Experience: Graduation from a college or university accredited by the American Medical Association with a Doctor of Medicine degree or accredited by the American Osteopathic Association with a doctor of Osteopathy degree. If medical education occurred at a foreign medical school, the applicant must possess a current Educational Council Foreign Medical Graduate (ECFMG) certificate. Successful completion of an approved program of residency training in family practice. Board certification in Family Practice preferred.

Three years of clinical experience in one or more areas of medicine, three years of administrative or supervisory experience, preferably in an ambulatory setting.

Documented experience in protocol development, evaluation and implementation of clinical standards of care based on the quality improvement process.

Documented experience as a preceptor for new personnel.

Documented experience as a consultant on special populations to health care providers and community agencies.

Experience in establishing and maintaining effective working relationships with other employees, patients and the general public.

CERTIFICATES, LICENSES AND REGISTRATIONS:

Active Illinois Physician licensure, plus current DEA registration, Board certification in a specialty area (must be completed within two years of employment), documentation of a Medical Degree from an accredited school of Medicine, documentation of residency completion in a specialty area from an accredited residency program and Physician National Data Bank verification of legal claims history.
