Job Description
A local company is seeking a Social Service Coordinator who will assume responsible for service management, developing relationships with service providers and agencies, educate residents on available services, monitor provisions of services, and work with the Property Manager as part of the management team. This is a short-term 5-week assignment, and you will work Monday through Friday from 8 a.m. to 4 p.m. Job Responsibilities: Monitor the delivery of services to residents to ensure they are appropriate, timely and satisfactory Perform service management function for all residents needing assistance, provide limited case management for a resident, and report all suspected abuse situations to the appropriate agency Advocate and negotiate on behalf of residents for adequate, timely, and cost-effective provisions of services and meet with service providers as needed Assemble a directory of community services and make it available to residents, families, and management, and contact hospital discharge planners to ensure residents receive appropriate services, and may attend discharge-planning sessions when appropriate Assist resident and family when a higher level of care may be necessary, assist and advise residents and families of the services that may be necessary, and assist residents in building informal support networks among themselves and with family members Act as liaison between community agencies, service providers, and residents, encourage residents to be proactive in meeting their social, psychological, and physical needs, and facilitate meeting of needs, when necessary, but avoid the creation of unhealthy dependence May assist residents in understanding lease and tenancy obligations, use the least drastic intervention necessary to alleviate a problem situation Educate residents on service availability, application procedures, resident rights, etc., both individually and as a group and promote wellness activities for all residents Document contacts with residents, providers, and families, maintain individual files on residents, keep files in a secure area to ensure confidentially, and maintain records in AASC database for all residents Work as a team member with the Property Manager in serving the residents, follow up on resident situations the Property Manager believes may need supportive services, and assist in identifying residents who need assistance Attend workshops and seminars on service coordination, housing, aging and computers to keep skills current and perform other duties as assigned
