Magnolia Post Acute Care

Position: Social Services AssistantLocation: Magnolia Post Acute Care in East County San DiegoStarting Pay: $20 to $24/hr DOE

Call or text Eddie at (808) 493-8454 for an immediate reply.

Shift:

  • Sunday thru Thursday 9am to 5:30pm

Responsibilities:

  • Function as patient advocate, helping ensure they have access to services, connecting them with community resources
  • Completing quarterly assessments for long term patients
  • Supporting discharge planning
  • Establishing firm boundaries with patients with goal of helping them achieve maximum autonomy and independence
  • Previous SSA or SNF experience required
