Springfield Suites Rehab & Nursing Center is seeking a full-time Social Services Coordinator to join our skilled nursing team.In this role, you will oversee social services, admissions support, and discharge planning while serving as a key resource for residents and their families throughout their stay.Salary: $45,000 per year
Responsibilities:Complete social services assessments, including BIMS, PHQ-9, and trauma screenings
Review admission documentation for appropriateness of placement
Coordinate discharge planning and aftercare services
Arrange home care, medical equipment, follow-up appointments, and community resources
Communicate with residents and families regarding concerns and care needs
Coordinate care plan meetings
Maintain advanced directives and resident records
Document assessments and services in PointClickCareQualifications:Experience in social work, admissions, and/or discharge planning in a nursing home or long-term care setting required
Knowledge of the Medicaid process required
Degree in Social Services, Human Services, Social Work, or a related field preferred
Strong communication, organization, and problem-solving skills
