Job Description

Social Services Coordinator- Housing

Full Time | Day Shift working 8:30 am- 5 pm

Pay between $20.44-22.99/hourbased on experience

Requirements for a Social Services Coordinator:

  • Education: one of the following required -
    • High School Diploma or equivalent with significant work experience
    • Some college coursework
    • Associate's degree
  • 3 years of experience in services for the elderly, disadvantaged or social services in a health care or related setting.
  • 5 years of experience in services for the elderly, knowledge of entitlements programs, health care and dementia issues preferred

Responsibilities for a Social Services Coordinator:

  • Connect residents and home-based participants with supportive services to promote successful aging in place.
  • Assist residents and families with transitions to higher levels of care, coordinating with outside agencies as needed.
  • Develop and maintain a comprehensive community resource guide.
  • Monitor and prioritize support for frail residents through regular contact and post-hospital/therapy follow-up.
  • Track and report service outcomes, including monthly and annual reports as required.
  • Develop and distribute communication materials (newsletter, website updates, social media, marketing content).
  • Serve as liaison to resident groups, families, and community partners.
  • Plan and coordinate social, educational, spiritual, and vocational programs to reduce isolation and support resident well-being.
  • Maintain a monthly activity calendar and support development of congregate and HCBS programs.
