Job Description
Greenup Rehabilitation & Nursing is hiring a Social Services Director to lead admissions, discharge planning, and resident support services in our skilled nursing facility.
If you have strong LTC experience and know how to move residents smoothly through admission to discharge-this is your role.
What You'll Do:
- Review and approve admissions for appropriate placement
- Complete BIMS, PHQ-9, and social service assessments
- Drive discharge planning (housing, services, equipment, follow-up care)
- Serve as a key contact for residents and families
- Lead care plan meetings and coordinate interdisciplinary care
- Maintain advanced directives and accurate documentation (PointClickCare)
What You Need:
- Nursing home or long-term care experience in social work, admissions, or discharge planning
- Strong understanding of Medicaid processes
- Degree in social services or related field preferred
