Social Services Director

$75,000-$90,000 per year (DOE)
Join Our Team as a Social Services Director!
McKinley Park Care Center is seeking a dynamic, compassionate, and organized Social Services Director to join our team. This role is ideal for someone who thrives in a fast-paced healthcare environment and is passionate about supporting residents and their families through every stage of care.
What You'll Do
Develop individualized care plans by working closely with residents and their families
Support residents in achieving the highest level of independence and well-being
Provide medically related social services to enhance residents' physical, mental, and psychosocial health
Assess emotional, psychological, and social needs of residents
Offer counseling and emotional support to residents and families
Assist with healthcare decision-making and coping with illness, disability, or loss
Coordinate discharge planning with interdisciplinary teams and outside agencies
Connect residents with community resources and support services
Schedule outpatient appointments and coordinate ancillary services (optometry, podiatry, dental, psychiatric, etc.)
Maintain accurate and timely documentation
Manage resident belongings and respond to lost/missing item reports
Provide staff in-service training as needed
Build strong relationships with internal teams and external partners
What We're Looking For
Bachelor's Degree in Social Work or Human Services required
Minimum of 2 years of supervised social work experience in a healthcare setting
MSW preferred
Strong communication, organization, and problem-solving skills
Ability to handle sensitive situations with professionalism and confidentiality
A proactive, compassionate approach to resident care
