Seeking a highly dedicated and experienced Social Services Director on behalf of a large Skilled Nursing / Post Acute Care Center. The successful candidate will be highly engaged in the care and discharge planning process for all skilled admissions at this high clinical-acuity center. In an environment of interdisciplinary collaboration, completes resident assessments, mediates family concerns, organizes safe transitions.

Job requirements:

  • Bachelor’s degree in Social Work or related field.
  • Three years of experience as a Social Services Director in post-acute settings.
