Requirements:

• LCSW
• BLS
• 5150 Training
• 2 years of experience in a healthcare setting, with at least 1 year in an acute care hospital
• Experience in discharge planning preferred

Job Description:

As a vital member of our team, you will:

• Conduct biopsychosocial assessments and crisis interventions
• Provide short-term counseling and facilitate support groups
• Coordinate care transitions and link patients with community resources
• Offer psycho-education and consultation to the interdisciplinary team
• Support patients in adjusting to illness and facilitate care goals discussions
• Participate in staff education and hospital committees

Skills and Knowledge:

• Strong clinical psychosocial skills and understanding of human behavior
• Effective discharge planning and knowledge of Medicare, MediCal, and regulations
• Familiarity with abuse reporting requirements

Job Accountabilities:

• Evaluate referrals within 24 hours
• Establish therapeutic relationships with patients and families
• Conduct psychosocial assessments and treatment planning
• Facilitate discharge planning for complex cases
• Refer patients to appropriate care and community programs
• Collaborate with healthcare team and community providers
• Maintain accurate patient records and community resource files
• Be available for scheduled coverage and on-call duties
• Evaluate patients for 5150 holds and arrange transport if needed
• Facilitate group therapy sessions as required
