Position Overview:

The Social Services Technician (Social Worker) role supports Sutter Sync HF patients by identifying and addressing social, behavioral, access, and caregiver barriers that affect adherence and outcomes. This role focuses on medication and device affordability, transportation, housing stability, food access, social isolation, caregiver support, and behavioral-health referral pathways when needs exceed digital or coaching support. This role is expected to be onsite at the Sutter Innovation Center at least 2 days per week.

Job Description:

These Principal Accountabilities, Requirements and Qualifications are not exhaustive, but are merely the most descriptive of the current job. Management reserves the right to revise the job description or require that other tasks be performed when the circumstances of the job change (for example, emergencies, staff changes, workload, or technical development).JOB ACCOUNTABILITIES:

  • Routinely monitors changes and participant and family needs, documents outcome measurements and advises the individual plan of care under the direction of the Medical Social Worker.

  • Provides social work interventions as prescribed by the individual plan of care and emerging needs which includes acting as a liaison to the family/caregivers, individual and group counseling, service coordination and discharge planning.

  • Prepares documentation monitoring participants progress in the adult day health care (ADHC) such as flow sheets, progress notes and reports to other service providers.

  • Conducts program tours, and assists in community outreach efforts and the family caregiver support program.

  • Participates in the daily activities of the program as needed including ambulating and transferring participants, pushing wheelchairs.

  • Facilitates support groups for participants or caregivers. Provides support, information, and referrals that allow participants to maximize their abilities as caregivers.

  • Actively works in support of regulatory compliance assuring that affiliate maintains full compliance with state and federal regulations. Understands specific responsibilities and ensures daily requirements are met.

EDUCATION:Equivalent experience will be accepted in lieu of the required degree or diploma.Bachelor's: Social Sciences, Social Work, Gerontology or related fieldsTYPICAL EXPERIENCE:2 years of recent relevant experience.

SKILLS AND KNOWLEDGE:Knowledge of community resources.Knowledge of individual and family development over the life span, and the influence of cultural and spiritual values in health care.General knowledge of illnesses, current treatments, and their physical and psychosocial effects on the individual.Knowledge of applicable laws and regulations, government and insurance benefits.Excellent interpersonal skills and time management skills, ability to work effectively in a fast paced environment, with rapidly shifting priorities and competing demands.Demonstrated ability to work independently, manage projects/tasks and deliver results on time.Communication skills, both written and verbal.PHYSICAL ACTIVITIES AND REQUIREMENTS:See required physical demands, mental components, visual activities & working conditions at the following link: Job Requirements

Pay Range is $39.45 to $55.23 / hour

The compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs.
