Job SummaryPerforms 100% intake and psychosocial assessment on all patients and their families admitted over the weekend. Facilitates case management function with coordinators for third party issues and communicates anticipated follow up needs with the primary SWCM team via email.Essential Functions
- Interviews all patients and family members, assesses their individual situations with regard to psycho-social situation and emotional adjustment to illness, formulates a clinical impression, and projects a discharge plan.
- Orients patient/family to Burke programs, policies, personnel, team members and rehabilitation philosophy and practice.
- Communicates effectively and works collaboratively with medical, nursing, clinical services, and insurance case managers to provide comprehensive medical care.
- Defines and interprets social/emotional material to the physician and other members of the health team. Provides consultation on emotional, social and financial aspects of care to hospital staff.
- May be asked to assist with a discharge. Provides resources and referral information as needed.
- Communicates information regarding pt’s progress to appropriate persons (i.e. designated discharge planners, insurance case managers)
- Attends and participates in educational programs for professional development.
- Identifies opportunities for continuous quality improvement.
- Maintains confidentiality and complies with professional ethics according to professional and division standards.
- Documents in the medical record according to division/hospital policy.
- Develops and maintains positive staff relationships within the program.
- Understands and follows division and hospital policies. Participates in departmental problem solving activities.
- Responsible for finding coverage if unable to keep schedule as agreed.
- Performs other duties as directed by the SWCM Director, SWCM Supervisor and Nursing Supervisor.
- Demonstrates the ability to translate knowledge of physical psycho-social and cognitive perceptual changes related to age into assessments, treatment plans and treatment modalities for pediatric, adolescent, young adult, adult and geriatric patients.
- Practices universal precautions.
- Personally adheres to Service Excellence standards by demonstrating positive communication, prompt & respectful service and maintaining an appropriate work environment when working with patients, staff, co-workers, visitors and others.
- Other duties as assigned.
Qualifications
Education & Experience Required: MSW, LMSW in New York State. Medical experience preferable.
Knowledge, Skills & Abilities: For weekend workers, responsibilities for 10-15 admissions/day and completing psychosocial assessments.
Work Environment: Considerable walking. Must be easily accessible by phone or email while on site. Office is on clinical unit in high traffic area with regular interruptions. Proximity necessary and desirable.
Special Requirements: Ability to interact with varied backgrounds of both professionals as well as patients and families. Must be skilled in communication and be able to deal with concepts in language of the listener. Ability to mediate and make assessments. Coordinate in objective fashion. Embodies essence and intent of Service Excellence.
The required per-diem coverage is Sundays, for up to 8 hours per shift.
