(3) Full-time openings (Permanent)- Social Workers (Float) M-F, 8hrs with weekend rotation (every 3rd/4th weekend).Requirements:

MD LMSW or higher
1+ years of discharge planning experience in an inpatient setting
Must have EPIC experience
