THIS HIRING MANAGER IS LOOKING FOR HIGHLY EXPERIENCED CANDIDATES WITH ACUTE INPATIENT CASE MANAGEMENT AND DISCHARGE PLANNING EXPERIENCETraining on unit will be minimal - candidates will need to be able to pick up quickly and work efficiently.

REQUIRED:

ACTIVE License - LMSW or LCSW AND ACTIVE MS State License
Minimum 2 years of Acute Inpatient Case Management experience that includes at least 1 year in a LARGE Hospital (500+ beds, LEVEL 1/ACADEMIC Medical Center) (this does NOT include telephonic, utilization management, or therapy experience),
Previous travel in a level 1 facility
Strong computer skills, Epic EMR experience,
Acute care/ED discharge planning experience, Recent (within the last two years) acute inpatient case management experience.

Shifts & Scheduling36/48 rotating schedule: 7a-7p every Wednesday & Thursday and every other Friday, Saturday, SundayWeekend Requirements NoneHoliday Requirements Rotated as neededCase load/# patients per day/consults per day 15-20

INTERVIEW PROCESS - leaders interview via webex (on screen interview) Please provide SPECIFIC times candidate is available to be on camera for a 20-30 min interview for the 72 business hours following submission.

UMMC has contracts with 5 local facilities with 10 beds reserved at each facility to assist with the complex placements, however specific criteria must still be met. Criteria has a limitation on financial assistance for only those uninsured or underinsured.UMMC is the only level 1 trauma hospital in the state so will receive a variety of complex patients and those with no other hospital options.

Scrub Color/Dress Code Any color scrubs or business professional attireRT for DC Planning? Will consult with RT as needPharmacy for DC Planning? Will consult with Pharmacy as neededDepartment Specific Orientation 2 weeks (will include general facility orientation) Will work alongside CM team so will never be working alone.
