Medical Social Worker – Home Health (Per Diem)CareCo Medical Home Health
CareCo Medical Home Health is seeking a Social Worker to provide in-home social work services to patients in the community. This per diem role is an excellent opportunity for a social worker who enjoys working independently while collaborating with a multidisciplinary care team.

Position Summary
The Social Worker addresses the social, emotional, and environmental factors that impact a patient's ability to achieve the goals of the Plan of Care. Services are provided in collaboration with nurses, therapists, and other care team members.
Pay and Schedule
- Per diem position
- Paid per completed visit and documentation
- Flexible work schedule
- Mileage paid between patient visits
Key Responsibilities
- Assess the psychosocial status of patients and identify barriers to care
- Complete assessments and documentation in a timely manner
- Develop and implement care plans in collaboration with the interdisciplinary team
- Provide counseling, guidance, and support to patients and families
- Educate patients, families, and team members on available community resources
- Initiate referrals and assist with discharge planning
- Participate in care conferences, team meetings, and quality improvement activities
- Communicate changes in patient status to physicians and care team members
- Maintain compliance with agency, state, and federal standards
Qualifications
- Master of Social Work Degree from a CSWE-accredited program
- Experience in a healthcare setting or equivalent
- Home health experience helpful
- Strong written, verbal, and interpersonal communication skills
- Knowledge of community resources and social service systems
- Valid drivers license, reliable transportation, and current auto insurance
