Social Worker (Lcsw) - Co Responder Social Service Unit

This job exists due to a grant received by the City of Peoria to implement a co-responder model for mental health professionals to respond to calls with the Peoria Police for individuals that need the assistance of mental health professionals. This service will allow us to connect individuals and families to mental health services beyond their initial crisis that may have resulted in call to the Peoria Police DepartmentProof of Auto InsuranceLicensed Clinical Social Worker (LCSW) - Illinois Department of Financial and Professional Regulation (IDFPR)Master's Degree: Social Work (Required)Basic computer skills/ability to keyboard – completing documentation in the electronic and/or paper medical record. Ability to perform de-escalation techniques Use of usual and customary equipment used to perform essential functions of the position. Work requires travelRecognize, respect, and effectively deal with values and cultural beliefs of clients, their families, and community resources and referralsWorks with law enforcement, emergency departments, and other emergency personnel in crisis situationsProvides follow up case management as assignedDemonstrates understanding of Medicare Home Care benefit including eligibility, qualification for admission and services available.
