Position Summary:

The Orchard Cove Skilled Nursing Unit Social Worker provides emotional support to residents and their families during crucial life transitions such as admission, discharge, transitions through Orchard Cove’s continuum of care and the dying process. The Social Worker shall advocate for the resident and his/her family on an individual basis and in a group setting in an effort to best support “What Matters Most” to each resident, in turn allowing the delivery of care and services to be aligned to those values and preferences.  The Social Worker is part of an interdisciplinary Orchard Cove team that ensures that residents live in the right place, at the right time and receive the right care. The Social Worker will assist each resident in maintaining his/her highest practical level of physical, mental, and psycho-social wellbeing.

Following the lead of the Orchard Cove service delivery model and its strong commitment to Health and Wellbeing, the Commons Residence Social Worker will take the lead in ensuring all residents on the Skilled Nursing Community participate in our Health and Wellbeing Navigation platform.  Through this platform, each resident will have a meaningful conversation that identifies What Matters Most to them and helps shape their best day.  In conjunction with other Commons Residence team members including the Director of Nursing, the Director of Rehab and the Director of Life Enhancement, the Social Worker will play a leadership role as a Health and Wellbeing Navigator to ensure our residents live their best day every day.

  • Leadership skills as this position is a key leader on the Commons Residence
  • Clinical knowledge and an understanding of proper usage of a medical record
  • Able to function as an integral member of the clinical team

Position Responsibilities:

  • Provides emotional support to residents and families during times of transition; adjustment to life in a nursing home; acceptance of change in medical condition and end of life
  • Key facilitator of Health and Wellbeing Navigation platform that ensures each Commons Residence resident lives his/her best day everyday
  • Completes all admission and discharge paperwork with resident/family.  Coordinates admissions and discharges
  • Serves as an advocate for a resident/family in individual and group settings including resident council, care plan meetings and other meetings
  • Key communicator between resident/family/staff
  • Responsible for achieving census and payor mix goals
  • Mange and lead inquiry tours and phone calls related to the Commons Residence. Serve as an ambassador for questions related to life and services on the Commons Residence
  • Create and nurture relationships with community partners, and HSL-wide partners, with the goal of facilitating referrals to the Commons Residence
  • Leader within an IDT who oversees transition planning and conversations for residents throughout OC
  • Lead and document all baseline, annual and quarterly care conferences; review the chart, admission packet signatures, admission assessments, social histories, and confirm advanced directives.
  • Participate in care continuum meetings
  • Ensure compliance with preadmission screening and discharge notifications and referrals
  • Admission and Bed Utilization: Point person with the Orchard Cove care continuum team and with the marketing team to leverage prospective resident requests across the different service lines of HSL.
  • Serves as Case Manager for all Independent and Enhanced Living residents who are hospitalized and/or are in Rehabilitation Centers. Responsible for all communication and resource finding with/for IL/EL residents of OC who are in the hospital and/or rehab

Qualifications:

  • Bachelor’s degree in Clinical Social Work (Masters Preferred)
  • Licensed Independent Clinical Social Worker (LICSW)
  • Excellent communication skills
  • Trained mediator
