Job Description

Ivy Park Post Acute is a 150-bed post-acute care facility located at 5609 5th Ave., Pittsburgh. Here, we believe our staff members are like plants. They need attention and effort to grow. Successful candidates will have the following:

degree in Human Services or similar field
experience in a SNF
compassion for a vulnerable population

Your day to day:

You'll provide assist with admissions and discharge planning, ensuring our residents' needs will be met. You'll work under supervision of our Social Services Director to ensure compliance with current state, federal, and local standards.
