Position Summary
Under general supervision and following prescribed policies and procedures of the health systems, interviews patients to obtain psycho social information and assists patients in resolving difficulties which interfere with obtaining maximum benefit from medical care. Performs social work interventions including assessment, counseling, case management and advocacy to maximize patient and family functioning and facilitate continuity of care. Provides consultation to the healthcare team regarding the impact of psycho social factors on health and illness. May provide social services to a patient population that includes ages ranging from newborn through geriatric.
Responsibilities
- Conduct assessments and provide social psychotherapy, counseling and psychosocial intervention with patients and families.
- Provide case management across the continuum of care, including discharge planning.
- Record assessment, interventions, progress in the medical record.
- Refer patients to community resources, contacts with various community agencies.
Requirements
- Currently licensed as a social worker or independent social worker in the state of Ohio.
- Ability to identify and plan for resolution of psycho social problems; gather/interpret data from multiple sources using professional level knowledge.
- Maintain discretion, provide counseling in emotionally-charged situations, utilize wide variety of professionals, communicates orally and in written form, use modern technology.
