Who You Are:

We are seeking a candidate with a strong positive attitude toward customer service to act as a Social Worker. This person will have a genuine interest in geriatric care and be able to motivate while maintaining relationship with staff and residents. The right candidate will always strive to do the best job possible, to be creative and to work towards accomplishing facility goals.

Position Summary:

Plans, organizes, develops, directs, and evaluates the resident social work function and the admission function in accordance with current applicable Federal, State and local standards. Coordinates discharge planning. Assists patients and their families to achieve and maintain maximum functioning in and outside of the community, through needs assessment, crisis intervention, counseling, and information and referral, and other miscellaneous duties as needed. Serves as the primary grievance official in the Health Center.

Essential Functions:

Entering an initial assessment note in the Social Service section of the chart within 72hours of admission, completing assigned sections of the MDS, completing and entering the Social History within fourteen (14) days of admission and re-evaluating the resident on a quarterly basis (sooner when a significant change occurs). These assessments are used to contribute to the resident care plan.

Contributing as an integral member of the interdisciplinary Resident Care Plan Conference. This also involves stating the social/emotional needs related to the resident's illness/disability, adjustment to placement, supportive network and response to treatment. The social worker makes specific recommendations to assist in the resident's overall care and genuine well-being.

Writing progress notes as least quarterly, earlier when there is a notable change. Each note should document progress made towards "social service" oriented goals, provide significant information about the individual and serve to communicate with the entire interdisciplinary team.

Initiating and/or participating in the written discharge plan which states the resident's specific need to be in the facility and if the resident is expected to be able to function in a more independent setting. The discharge plan should include consultation with otherdisciplines, the family and of course, the resident (whenever possible).

Participating in the post D/C plan as needed or assigned.

Attending Resident Council meetings as a liaison to listen to resident concerns and clarify issues and/or facilitate changes (when invited by the Resident Council governing body). The Social Worker should be involved with the Activity Department and assist with the Resident Council meetings as necessary. The social worker is a resident advocate, as well as a liaison between the resident and his/her family, the facility and community agencies.

Contacting community agencies for the purpose of resident referral. These agencies include, but are not limited to: the Department of Mental Health and Developmental Disabilities, Department of Public Aid, Veteran's Administration, community mental health centers, transportation companies, social clubs, etc.

Assisting the resident to participate in individual and group programs and to utilize these programs to full advantage.

The social worker should promote resident rights issues, including: self-determination, treatment of resident with respect and dignity, the right to voice grievances, and the right to meet with other residents.

The Social Worker will be instrumental in providing Quality Assurance services. This may be accomplished with formal and informal QA studies/audits.
