About Position: The primary responsibility of the Social Worker is to provide social work care management services to the participants of the Piedmont Health SeniorCare program; provides services such as participant screening, assessments, care management, supportive listening and coaching and referrals.
- Work Location: 1214 Vaughn Rd, Burlington, NC 27217 (Piedmont Health SeniorCare in Burlington)
- Schedule: Monday through Friday, 8:00am - 5:00pm
Education/Experience: This position requires a master’s degree from an accredited school of social work, 2 years professional level experience in a health related area. One year of experience with the frail or elderly preferred.

Licensure, Registry or Certification Required: Master of Social Work (MSW).
Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.
Pay Range: $59,050 - $79,358 (commensurate with years of experience)
