Pay Range (depending on experience):
$33.42 - $49.30.
Job SummaryAs a member of a multi-disciplinary team, you will be responsible for assessing and providing support and intervention for the social and emotional needs of patients/residents and their families to promote continuity of care and services. Work includes conducting psychosocial assessments and interventions, mental health screens and referrals and discharge planning for Acute Care patients, Swing Bed residents and patients in Emergency Department.Job Duties and Responsibilities1. Work collaboratively with providers, nursing staff and CNA’s, therapy department, pharmacists, clinic case managers, community resources and agencies.2. Communicate patient needs to assist treatment team in advancing patients care and discharge goals. Develop discharge plan through collaboration with interdisciplinary team.3. Meet face to face with patients/families/patient representative to assist with discharge planning and cross continuum of care.4. Conduct psychosocial assessments and mental health screens in Emergency Department and assist with appropriate referrals of care i.e., contacting community providers to establish continuity of care, referrals to Designated Crisis Response Team and/or facilitate In Patient Psychiatric Admissions.5. Collaborate with internal and external resources to facilitate a safe transition from hospital to another facility or home with appropriate services. Identifies discharge barriers and initiates action to resolve.6. Maintain timely documentation in patient/resident electronic record.7. Performs special projects and other related duties as assigned.8.
