The Department of Psychiatry has an exciting opportunity for a full-time Social Worker to work in Miami, Florida.

Department Specific Functions

  1. Advocates for and helps clients gain access to resources while confronting their personal issues.

  2. Refers patients and their families to specialized treatment programs in accordance with their needs, and monitors and evaluates their progress through the Behavioral Health Hub programs.

  4. Will support the UM BHH Pediatric site with comprehensive practice transformation services.

a. This includes training, technical assistance, telepsychiatry consults (e.g. psychiatrists and psychologists), and care coordination/management to advance quality improvement, patient-centered care, and characteristics of high performing primary care including the integration of Behavioral Health Services.

  1. The Social Worker will be a key contact for the Florida Department of Health Behavioral Health grant. The incumbent will have an integral part in the program development under the Program Management team and will be responsible for the adherence of the BHH contract deliverables. The incumbent will also serve as the liaison with pediatric primary care centers, behavioral health organizations, and other mental health stakeholders in the communities within the State of Florida.

  2. The Social Worker will attend the FL DOH collaborative meetings.

  3. The Social Worker will meet with potential pediatric providers, and meetings to network with behavioral health organizations.

  4. The Social Worker is responsible for BHH referrals, triage and assessing social services intakes in addition to serve as a liaison for provider-to-provider consultations. The incumbent will assist patients and the families to follow treatment recommendations.

  5. The Social Worker will recommend training topics for the UM Behavioral Health Hub. 10. Monitor referrals via the BHH inbox (bhh@miami.edu), EPIC Ambulatory referrals, and follow up BHH referrals within 48 hours of referrals.

  6. Social Worker will initiate social services assessments including GOT Transition Assessment Survey and PSC-17 assessment forms.

  7. Responsible for updating logs and reports required to submit withing the BHH Tasks and State Deliverables

  8. BHH Development and Maintenance:

  9. Medical Records Management: Create and maintain medical records for all clients receiving BHH services through the contract requirement, ensuring compliance with confidentiality and regulatory standards.

  10. Assist Program Manager completing enrollment agreements:

  11. Maintain relationships with participating providers to ensure the minimum number of client referrals as specified in the contract.

  12. Update Quarterly Participant Roster:

  13. Complete the BHH participant Roster each quarter and submit it as required.

  14. Evidence-Based Training: Conduct at least one evidence-based skill-building training each quarter for enrollees to enhance their competencies.

  15. Submit to enrolled providers Skills Assessment feedback forms.

  16. Client Satisfaction Assessment:

  17. Determine each enrollee's satisfaction with the BHH's services by administering the Primary Care Provider (PCP) Satisfaction Survey, collecting completed surveys, and analyzing the results to complete the BHH Participant Satisfaction Summary Report.

  18. Transformation Plans:

  19. Develop, implement, and update a Transformation Plan for each enrollee based on the results of their skills assessments within 150 calendar days of enrollment.

  20. Client Referral Metrics:

  21. Ensure a minimum of 60 clients are referred to the BHH each quarter, completing the Client Roster for each referral and submitting it with the Quarterly Progress Report.

  22. Client Satisfaction Surveys reviewed with Program Manager:

  23. Measure client satisfaction with BHH services by providing a Client Satisfaction Survey one month after referral and every six months or prior to discharge.

  24. Quarterly Reporting:

  25. Complete the Statewide Pediatric Behavioral Health Collaborative Summary Report Template and the Quarterly BHH Data Report Template each quarter.

  26. Quality Improvement Initiatives:

  27. Identify, design, and implement a BHH quality improvement initiative related to pediatric behavioral health hub.

  28. Statewide Initiative Participation:

  29. Collaborate with the Department and other Regional BHHs to identify and participate in the implementation of one statewide quality improvement initiative.

  30. Annual Data Reporting:

  31. Consolidate data from the Quarterly BHH Data Report and use it to complete the Annual BHH Data Report.

  32. Data Submission based on assigned deliverable dates such as Quarterly and Annual Progress Reports.

Minimum Qualifications

Education:

Bachelor's degree in Social Work, Psychology, or related field

Experience:

Minimum 1 year of experience. Any relevant education, certifications and/or work experience may be considered.
