Responsibilities Include:Assessing patients' social, emotional, and financial needs

Assisting with community resources and discharge planning

Providing counseling and emotional support to patients and families

Collaborating with physicians, nurses, and therapy staff to ensure quality patient care

Advocating for patient well-being and independence Qualifications:Licensed Social Worker (LSW) required; MSW preferred

Home health experience preferred, but not required

Excellent communication and organizational skills

Reliable transportation and valid driver's license
