Job Description
Job Title: Medical Social Worker (Home Health)
Join Our TeamAre you a compassionate advocate who thrives in a collaborative environment? We are looking for a dedicated Medical Social Worker to join our home health team. In this role, you will be the bridge between our patients and the resources they need to thrive at home, ensuring their social and emotional needs are met alongside their clinical care.
What You’ll Do
- Patient Advocacy & Assessment: Assess the psychosocial status of patients in their home environment and communicate critical findings to the RN and clinical team.
- Care Planning: Develop and carry out social evaluations and intervention plans tailored to each patient’s unique illness and living situation.
- Community Liaison: Serve as the primary link between patients/caregivers and community agencies, providing essential referrals for practical and environmental needs.
- Collaborative Care: Participate in discharge planning, case conferences, and the development of the total plan of care to support the patient’s health goals.
- Consultation: Assist physicians and the medical team in understanding the social and emotional factors impacting a patient’s recovery.
Qualifications
- Education: Bachelor’s degree in Social Work from a CSWE-accredited program (Master’s or Doctoral degree preferred).
- Licensure: Must be licensed as a Licensed Clinical Social Worker (LCSW) by the Board of Behavioral Sciences.
- Experience: Minimum of one (1) year of healthcare experience. Home health experience is strongly preferred.
- Skills: Excellent verbal/written communication and strong organizational skills.
- Requirements: Must possess current CPR certification, a valid driver’s license, and a reliable, insured automobile.
