Job Description
- Develop care plans for each assigned client.
- Coordinate services for socially and medically complex clients.
- Monitor health and/or psychosocial changes in clients through monthly contacts with client, family, and professionals.
- Participate in case consultation with the nurse care manager.
- Arrange medical/social services for clients within budgetary and contractual guidelines.
- Master’s Degree in Social Work, Psychology, Counseling, Rehabilitation, Gerontology or Sociology plus one year working with the elderly; or
- Bachelor’s Degree in one of the above fields, and two years experience working with the elderly.
- Position requires extensive client contact by phone and in person.
- SWCM must have ability to communicate in English, in both written and verbal forms. Bilingual Spanish is highly preferred due to client population.
- There will be frequent travel to clients’ homes throughout West, Central, and East Contra Costa County.
- Must have valid California Driver’s license, clean DMV record, and automobile insurance.

Over the years, the organization has provided a wide variety of programs, with the goal of promoting dignity and independence of people with disabilities and special needs. Our mission is to create opportunities where people can learn, grow, and age independently with dignity in community.Program HistoryAdult Day Health Care (ADHC) is a program that was created in California in 1978 in response to a need to have an alternate solution to institutionalization. The program mirrors the skilled nursing staffing model, but is based in the community and typically operates during the day on weekdays. It is akin to a senior center for those who can no longer attend a senior center independently. It is especially beneficial for those who need additional support to do things like ambulate, eat, toilet, participate in activities and more. Staff provides therapeutic activities disguised as fun throughout the day, making socializing and exercising meaningful and pleasurable. This makes folks want to come back each day!
