The Role at a Glance

Reporting to the Assistant Director of Social Impact, the Community Liaison serves as a trusted resource for households participating in the GENESIS program. You'll coordinate services, connect families with community resources, mentor medical students during community-based learning experiences, and collaborate with interdisciplinary teams to improve the health and well-being of underserved populations.

What You'll Do:

  • Build trusted relationships with individuals and households while conducting assessments and coordinating services that address social determinants of health.
  • Connect families to healthcare, behavioral health, housing, food, financial assistance, and other community resources.
  • Mentor medical students during household visits, helping them understand the real-world impact of social and environmental factors on health.
  • Collaborate with faculty, healthcare providers, and community organizations to improve access to care and strengthen community partnerships.
  • Support crisis intervention, advocacy, and care coordination for vulnerable populations.
  • Track household outcomes, maintain program documentation, and contribute to continuous program improvement using innovative population health technology.

What You'll Bring

  • Master's degree in Social Work (MSW) from an accredited institution.
  • Three or more years of experience in social work, care coordination, case management, behavioral health, community outreach, or a related human services field.
  • Experience serving underserved, vulnerable, or high-risk populations.
  • Strong knowledge of community resources, social service systems, advocacy, and crisis intervention.
  • Excellent relationship-building, communication, and interdisciplinary collaboration skills.
  • Licensed Social Worker (LMSW/LCSW or equivalent) and bilingual English/Spanish or Tagalog are preferred.
