Job DescriptionATC Healthcare is seeking a licensed Social Worker for a travel assignment providing behavioral health and case support services in a secure care setting. The Social Worker will manage a daily caseload, conduct assessments, coordinate services, and support patient care planning. Candidates must hold an active license (LCSW or LMSW) at time of submission. Schedule is primarily weekday day shifts, with possible weekend coverage depending on assignment needs.

Key Responsibilities

• Provide social work and case management services to assigned patients
• Conduct psychosocial assessments and care planning
• Manage a caseload of approximately 10–15 patients per day
• Coordinate referrals and community resources
• Support discharge and continuity of care planning
• Document encounters and plans in required systems
• Collaborate with interdisciplinary care teams
• Maintain confidentiality and compliance with policies

Qualifications

• Active LCSW or LMSW license required at submission
• Social work experience in clinical or institutional settings preferred
• Ability to manage moderate daily caseloads
• Strong assessment and care coordination skills
• Ability to work 8-hour shifts or approved 4x10 schedule
• Weekend availability as needed by schedule
• Resume and required onboarding forms required at submission
• Government ID and background screening required
