South East Chicago City with focus on 60617, 60619, 60620, 60628 AND 60643 communities. Weekdays 8-5. M-F. Weekend and holiday rotations.

Pay Range

$30.70 - $46.05

You will support the mission of Advocate Home Health Services by delivering cost-effective, outcome-focused, and patient-centered care. Assesses patients’ psychosocial needs in the home setting, develops and implements care plans to address those needs, and collaborates with the care team to coordinate services and promote positive patient outcomes.

Advocate Health at Home provides compassionate medical care and services – all in the comfort, safety and convenience of the patient’s home.  Advocate Health at Home helps people of all ages – from infants to those seeking end-of-life care. Some people need care and services for just a short while, such as following surgery. Others need help long-term to manage a chronic illness or navigate care at end of life.  Advocate Health at Home provides comprehensive, quality home health services throughout the Chicago area.

We’re Looking For:

Licensure or Certifications

  • License/Registration/Certification:  IL locations-LSW Eligible, LCSW preferred issued by the State of Illinois
  • WI locations -Social Work Certification issued by the State of Wisconsin

Education/Experience:

  •  Masters of Social Work services from an accredited school
  • Years of Experience:  1-2 years of experience in a clinical setting post licensure preferred.
  •  Basic proficiency with computers, including Microsoft Word and email
  • Ability to learn and effectively use electronic scheduling and documentation systems
  • Strong written and verbal communication skills with attention to professional documentation
  • Excellent interpersonal skills
  •  Effective time management and organizational abilities
  • A valid driver’s license issued by the Division of Motor Vehicles 

As a Medical Social Worker (Home Health) ,  you will:

  • Establish strong patient relationships through effective communication, individualized care, and patient/family involvement in care planning and self-management.
  • Ensure high-quality, safe, and cost-effective care using evidence-based practices, patient education, and thorough documentation.
  • Collaborate with interdisciplinary teams to coordinate care, manage changes in patient needs, and support smooth transitions and discharge planning.
  • Demonstrate professionalism through teamwork, ongoing learning, and effective communication while supporting organizational efficiency and responsible resource use.

Physical Requirements and Working Conditions

  • Extensive travel within geographical boundaries in all extremes of weather and on all types of roads including expressways.
  •  Must be able to provide/arrange own transportation to and from patients’ homes and branch office.
  • Ability to work rotating schedules as needed. Requires adherence to scheduled days and hours of work based on commitment to patient/client and workload assigned.
  • If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
