Job Description:

A Brief Overview

Addresses the social, emotional and financial needs of patients and their families as these needs relate to health care needs and concerns. Demonstrates the ability to work collaboratively as a member of the Home Health/Hospice Interdisciplinary Team. Demonstrates knowledge of process of death, dying, and bereavement required. Demonstrates understanding of the psychosocial factors for terminally ill patients and their families.

What you will do

  • Determine clients' eligibility for financial assistance.
  • Interview clients individually, in families, or in groups, assessing their situations, capabilities, and problems, to determine what services are required to meet their needs. Effectively communicate concerns and collaborate with management and other members of the interdisciplinary team.
  • Facilitate individual and/or group counseling sessions that provide support in such areas as grief, stress, or chemical dependency.
  • Lead community workshops related to grief and bereavement.
  • Provide bereavement counseling to families and community members suffering loss.
  • Refer clients to community resources for services such as counseling for complicated grief, job placement, debt counseling, legal aid, housing, medical treatment, or financial assistance, and provide concrete information, such as where to go and how to apply.
  • Serve on policymaking committees, assist in community development, and assist client groups by lobbying for solutions to problems.
  • Supervise other social workers.
  • Complete psychosocial assessment, care plans, and discharge plans on hospice patients and families.
  • Complete documentation timely and accurately per regulatory guidelines.

Qualifications:

Qualifications

  • Master's degree in Arts/Sciences (MA/MS) Master's degree in Social Work from a program accredited by the Council of Social Work Education Required
  • 1-3 years Social Work experience in a health care setting Preferred
  • Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders.
  • Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.
  • Knowledge of group behavior and dynamics, societal trends and influences, human migrations, ethnicity, cultures and their history and origins.
  • Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction
  • Talking to others to convey information effectively.
  • Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.
  • Monitoring/Assessing performance of yourself, other individuals, or organizations to make improvements or take corrective action.
  • Being aware of others' reactions and understanding why they react as they do.
  • Actively looking for ways to help people.
  • The ability to listen to and understand information and ideas presented through spoken words and sentences.
  • The ability to communicate information and ideas in speaking so others will understand.
  • The ability to tell when something is wrong or is likely to go wrong. It does not involve solving the problem, only recognizing there is a problem.
