Saint Luke's is seeking a weekend Social Worker to assist patients in an acute care setting.
Responsible and accountable for assessing the psychological social emotional cultural or financial needs that impact patient s health and recovery. To provide professional social work services that utilize supportive counseling and clinical knowledge in assessing the needs of patients and families during illness crisis intervention and post discharge planning. Collaborates with members of health care team in the development of a transition plan and addresses needs in a timely manner. Participates in program planning and development to meet the needs of customers within the health care system and community.
- Part time; Saturday & Sunday 8:00am-6:00pm; in-person
- Complete psychosocial assessments on admitted patients experiencing acute, chronic or terminal illnesses.
- Formulate discharge plans with patient preference and treatment team recommendations in mind.
- Collaborate with the care team
- This is a WEEKEND position. They will be working with a SW team of 2 both days and will need to be able to prioritize their day to best meet throughput. Our weekend team often discharges 40-50 patients on a weekend, so prioritization and critical thinking are a must.
- Master's degree in social work from a school of social work accredited by the Council on Social Work Education (CSWE)
- Current Missouri Social Work license (LMSW or LCSW)
- Prefer prior experience in a healthcare setting (i.e., hospital, home health or nursing home) but not required.
