Job Description
Job Title: Social Worker Employment Type: Contract Work Hours: 40 hrs./week Work site: 100% Onsite Location:134 Main Street, Saugerties, NY Job Duration: 2 Years Closing: 10/31/2025. Duties Description: Licensed social worker ( LMSW or LCSW) that will provide clinic services to youth in a school-based setting from 8 am
- 4:30 pm Mondays
- Fridays Maintain an average clinic caseload of approximately 25-30 students grades kindergarten
- 12th grade Participate in all admissions with child, family and telehealth MD through NYU Work collaboratively with our NYU telehealth fellows on each youth's treatment needs Provide Individual and Family Therapy and crisis intervention as needed Collateral involvement with Saugerties Central School District and any outside agencies involved in the youth's care Function as part of an interdisciplinary treatment team Participate in weekly Treatment Team meetings Advocate for youth and families to obtain community-based linkages with additional supportive services that meet their needs Maintain timely medical record documentation Facilitate thorough discharge planning and complete discharge follow-up Collaborate with school liaisons within Saugerties to facilitate a streamlined and efficient referral and screening process Minimum Qualifications: Master's degree in social work. Minimum 3-5 years of experience. Misc. Information: Shift timing for this role is usually 8 am to 4:30 pm Monday to Friday. Parking: Free Parking facility available at the job location. Public Transportation: No transportation facility is available; employees are required to have their own vehicle. Interview will be conducted in person or online based on the availability.
