Job Summary: Tryfacta is seeking a Social Worker for our client in Goshen, NY10924. This is a 2-year contract assignment. Position Title: Social Worker Location: Goshen, NY10924 Duration: 2 years Work Schedule: Monday-Friday 8:00am-4:30pm Pay rate: $42 Responsibilities for this position include, but are not limited to:

• Assess children and families, develop individualized care plans, and determine appropriate resources and support services.
• Advocate for individuals and families and assist them in accessing and obtaining needed services.
• Contact service providers and coordinate with community resources to facilitate appropriate services, placements, and referrals.
• Attend interdisciplinary conferences and collaborate with service providers and other professionals to determine appropriate placement and treatment resources.
• Monitor individual rehabilitation goals, service plans, and progress toward established objectives.
• Provide crisis intervention services and appropriate support to children and families during periods of crisis.
• Maintain communication and coordination with individuals, families, service providers, and interdisciplinary team members.
• Travel as required to provide services; a state vehicle will be provided for work-related travel.
• Complete required documentation and comply with applicable program and agency requirements.
• Participate in OMH-required training, which will be provided by the Office of Mental Health.
• Work Monday through Friday, 8:00 AM 4:30 PM, for an anticipated assignment duration of approximately 6 months, subject to change and not guaranteed.

To be considered for this position, you should have: [Skills, Education, or Experience]

• Bachelor's degree in social work (BSW).
• Ability to work under the supervision of a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW), as applicable.
• New York State LCSW/LCSWs qualification where required by the position.
• Valid driver's license and ability to travel as required for the assignment.
• Strong assessment, care-planning, advocacy, communication, and case-management skills.
• Ability to coordinate services and collaborate effectively with interdisciplinary teams and community service providers.
• Experience or demonstrated ability in crisis intervention and supporting children and families.
• Ability to monitor rehabilitation goals and evaluate individual progress.
• Willingness to complete OMH-required training provided by the agency.
