Speech-Language Pathologist (SLP) – Full-TimeEpsom Health Care Center | Epsom, NH$42–$46/hour | Full-Time | BenefitsPost Rehab is seeking a Speech-Language Pathologist (SLP) to join the rehabilitation team at Epsom Health Care Center. This full-time opportunity offers the chance to work closely with residents on communication, cognitive, and swallowing needs while being part of a supportive skilled nursing and post-acute care team.What You'll DoEvaluate residents with speech, language, cognitive-communication, and swallowing disorders

Develop individualized treatment plans based on clinical findings and resident goals

Provide skilled interventions focused on communication, cognition, and dysphagia

Monitor resident progress and adjust treatment approaches when appropriate

Educate residents, families, and caregivers on therapeutic strategies and recommendations

Complete accurate and timely clinical documentation

Collaborate with PT, OT, nursing, physicians, and other members of the interdisciplinary care team

QualificationsActive New Hampshire Speech-Language Pathologist license required

Graduate degree in Speech-Language Pathology from an accredited program

Strong clinical, communication, and organizational skills

Skilled nursing or rehabilitation experience preferred, but not required
