️ What You'll Do as a PRN Speech‑Language Pathologist

You'll play an essential role in delivering personalized, top‑quality home‑based therapy. Your impact includes:

  • Promoting Elara Caring's mission & high‑quality standards
  • Conducting SLP evaluations to determine patient needs
  • Completing OASIS-driven assessments and follow-up visits
  • Developing personalized Plans of Care
  • Ensuring patient safety and engaging family/caregiver resources
  • Communicating care updates with physicians and interdisciplinary teams
  • Documenting accurately & timely
  • Providing direct therapy using accepted, evidence-based practices
  • Teaching patients/caregivers home exercise programs
  • Participating in consistent care coordination

What You Need

  • Master's degree in Speech-Language Pathology
  • Current, unrestricted Indiana SLP license
  • Minimum 1 year of clinical experience
  • Ability to travel within the Terre Haute service area
  • Reliable vehicle, valid driver's license & auto insurance
  • Ability to lift 50-100 lbs and perform physical tasks as needed
  • Willingness to travel up to 50% depending on caseload
