A Brief Overview
The Patient Transport Coordinator plays a critical role in facilitating seamless patient transportation in collaboration with hospital staff and external partners. This position is responsible for coordinating interfacility transports and arranging transportation services for discharged patients. The Patient Transport Coordinator works closely with GRH transport departments, local EMS providers, and public transportation services to ensure timely and efficient non-emergent and emergent transport solutions.Key responsibilities include auditing and reviewing healthcare documentation to ensure compliance with medical standards and billing protocols. This role requires expertise in medical terminology and ambulance billing requirements to validate patient demographic and insurance information. The Patient Transport Coordinator ensures adherence to state and federal regulations by verifying the accuracy of CMS ambulance signature forms, Advance Beneficiary Notices (ABNs), and Non-Emergent Ambulance Transportation (NEAT) documentation, supporting both regulatory compliance and accurate billing processes.
What you will do
- Collaborates as part of a team in conjunction with hospital staff (such as the Access Center, Emergency Department, Discharge Planners, Social Workers) to coordinating patient transport services for SEIRMC and HCHC.
- Plans and directs transport requests to ensure they are fulfilled in a timely manner.
- Collaborates with referring facility to accommodate, arrange, and schedule patient transfer services.
- Uses knowledge of geography and service area to analyze transport times and calculate arrival times to ensure maximum efficient with available resoursces.
- Interact professionally while coordinating transfers with other health care professionals.
- Operates computers, software, and multi-line phone system.
- Ensures the provision of optimal transportation services to patients and referring agencies.
- Reviews, analyzes and determines opportunities for documentation improvement.
- Adapts to staff and customers to provide effective communication with physicians, staff, and leaders.
- Timely follow up on all cases.
- Manages and analyzes multiple tasks simultaneously to prioritize transfers.
- Evaluate patient care reports for completion for billing purposes and submit for billing
Qualifications:
Qualifications
- H.S. Diploma or General Education Degree (GED) Required
- 1-3 years Prior experience working in Dispatch, EMS, or in a hospital setting.
- Knowledge of medical terminology.
- Strong communication skills and ability to work in a team environment.
- Ability to deal with high stress situations.
- Able to quickly manage and transition between various tasks while maintaining attention to detail.
- Must be able to work independently; display exceptional initiative; and demonstrate effective problem-solving skills.
- Knowledge of value-based care measurements.
- Understanding of community services to assist with discharge planning.
- Gather and analyze information pertinent to prioritizing patient transports.
