The Supervisor, Case Management is responsible for the daily shift operations of the Care Management Department. In collaboration with the Nurse Manager, this role supports departmental operations, staff oversight, and delivery of high-quality, patient-centered care. In the absence of the Nurse Manager, the Supervisor assumes operational responsibility for the department. The Supervisor provides clinical leadership, operational coordination, and staff mentorship to ensure compliance with regulatory, organizational, and hospital standards.
Primary Responsibilities:
Operational & Clinical Leadership
Coordinates, supervises, and is accountable for the daily shift activities of the Care Management Department
Sets priorities for the team to ensure timely and effective completion of tasks and workflows
Coordinates work activities with other supervisors and interdisciplinary partners to support continuity of care
Develops and implements short-term operational plans to meet departmental objectives
Identifies, evaluates, and resolves operational issues using defined processes, clinical expertise, and sound judgment
Ensures delivery of quality care in accordance with regulatory requirements, hospital policies, and professional standards
Staffing, Scheduling & Performance Management
Under the direction of departmental leadership, assists with daily and monthly staff scheduling
Participates in the hiring, onboarding, training, and ongoing staff development of assigned team members
Supports performance management through coaching, mentoring, and feedback
Encourages professional growth and development of staff and fosters a collaborative team environment
Clinical Expertise & Education
Serves as a clinical resource, mentor, and educator for Care Management staff
Maintains current knowledge of professional nursing practice and participates in continuing education activities
Safety, Quality & Compliance
Maintains a safe environment for patients, staff, and visitors in compliance with NLH policies and procedures
Reviews, documents, and addresses incidents and safety concerns within the area of responsibility
Reports all lost-time work injuries within 24 hours to the appropriate HR or Safety contact
Promotes open, non-punitive communication related to patient, employee, and environmental safety concerns
Decision-Making & Scope
Decisions are guided by established policies, procedures, and business plans
Impact of work is typically at the team level, with accountability for task-level outputs
Product, service, or process decisions may affect individual employees and internal or external customers
Additional Duties
Performs other related duties as assigned to support departmental and organizational needs

Required Qualifications:
- Bachelor's Degree
- Maine Registered Nurse (RN) License, or eligibility to obtain
- 3+ years of relevant clinical or care management experience
- Proficiency with spreadsheet applications, including calculations, formulas, graphing, pivot tables, and macros
- Proficiency with presentation software, including use of multimedia, animations, and design elements
- Proficiency with word processing applications, including document formatting, tables, footnotes, table of contents, and mail merge functions
Working Conditions
- Ability to sit and work at a computer and telephone for 2-4 hour periods

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The salary for this role will range from $72,800 to $130,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
