Primary Care Physician (Remote | Full-Time | No On-Call) Monday- Friday 9am-6pm

Our Community Health Workers, each a licensed nail technician, provide in-home preventive foot care that builds trusted relationships and opens the door to better health above the knee. Through these recurring visits, Belle helps identify emerging health issues earlier, connects members to Chronic Care Management, and extends the reach of our clinical team through AI-powered telehealth and care coordination.

We’re looking for a Board Certified (or Eligible) Primary Care Physician to join our team and deliver high-quality, preventive-focused care through telehealth.

What You’ll Do
- Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management)
- Create and manage personalized care plans for ongoing conditions
- Collaborate with our integrated care team (nurse case managers, NPs, CHWs)
- Ensure thorough documentation in our EHR and compliance with quality measures
- Engage patients in preventive care to reduce avoidable complications
Requirements
What We’re Looking For
- MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
- Active medical license — multi-state licensure a plus
- Credentialed with major plans: UHC, Aetna, Humana
- Strong interest in preventive care, senior health, or chronic condition management
- Tech-savvy and comfortable practicing via telehealth
- Collaborative, mission-driven mindset with excellent communication skills
