Description: Physical Therapy Assistant for North Country Home Health & Hospice Agency. Location/Territory: Northern Territory - Lancaster, Groveton, Stratford, Stark, Columbia, Colebrook, Pittsburg, Errol. Candidates must have their own vehicle and be comfortable driving in winter weather.NO LOCALS - Must be 50+ miles from facility. Permanent address must be included in the submission file [REQUIRED]The North Country Home Health & Hospice Agency Physical Therapist Assistant is responsible for providing physical therapy treatments under the supervision of a qualified Physical Therapist in accordance with the plan of care. The Home Health Physical Therapist Assistant will utilize therapeutic exercise, modalities and activities to achieve the highest level of function possible within the home environment.RequirementDescription:
- 1-2 years current Physical Therapist Assistant experience in Home Health [REQUIRED]
- age group experience: Peds to Geri
- Previous travel experience [HIGHLY PREFERRED]
- Home Health and/or Hospice experience [REQUIRED]
- Work History
- Physical Therapy Assistant skills checklist, current within past year [REQUIRED]
- 1 Supervisory Signed Reference, from the past year of employment [REQUIRED]
- Active American Heart Association BLS - [REQUIRED][DOCUMENT]
- Active Physical Therapy Assistant practice license from the New Hampshire Office of Allied Health Professional [REQUIRED][DOCUMENT]
- License verification [REQUIRED][DOCUMENT]
- All licenses/certs must be current at time of submission
- Permanent address must be included in submission file [REQUIRED]
